Do you have concerns about your baby’s development? We can help! Through our Early Childhood Department, the Cornell Scott-Hill Health Center participates in the Connecticut Birth to Three system and accepts referrals through the Child Development Infoline. If you have concerns, please request a referral. Then, we will evaluate the need for services in all developmental areas – motor, cognition, communication, personal-social, and self-help skills. Early intervention services including speech therapy and occupational therapy are provided as needed. Services are delivered both at home and in the community.
To request Birth to Three early intervention services you can either:
a) Contact Infoline (211) or the Child Development Line directly (800-505-7000). If you would like our program to provide the evaluation, you can request us by asking for Cornell Scott-Hill Health Center at 226 Dixwell Ave, New Haven (203) 503-3440 at that time; otherwise, your child will be assigned to the program in your town that is next on rotation.
b) Follow the link: (http://referral.birth23.org/guestbook/ReferralForm.htm) to e-mail your request directly to info-line. You can designate Cornell Scott-Hill Health Center on the referral form.
